Effective October 26th, 2023 - I am an in-network provider with Anthem Blue Cross Blue Shield and Medical Mutual.

No Limits Occupational Therapy is primarily an out-of-network provider. I accept private payment via Venmo, cash, or check. I also accept county funding from Montgomery, Greene, and Warren County.

If I am an out-of-network provider with your insurance company, I can provide superbills monthly, upon request, for those who would like to submit to their insurance for retroactive reimbursement.

A superbill is a form completed by an occupational therapist or other medical practitioner that allows patients to be reimbursed directly from their health insurance companies.

A superbill does not guarantee that an insurance provider will pay for the services provided. Each insurance plan is different. Depending on the specific insurance policy, some policies do not offer any out of network benefits, while others offer nearly full coverage for out of network therapy sessions. It is your responsibility to contact your insurance provider and find out exactly what will be covered.
